<p><strong>Remote RN Care Coordinator (Part-Time to Potential Full-Time)</strong></p><p>We are seeking a dedicated and experienced <strong>Registered Nurse (RN) Care Coordinator</strong> for a fully virtual position supporting population health and care coordination efforts. Candidates must hold an active RN license in <strong>New York, Texas, or Oregon</strong>. This role will begin as part-time with the potential to transition into a full-time opportunity based on performance and program needs.</p><p>The ideal candidate will have prior experience working in an <strong>Accountable Care Organization (ACO)</strong> or similar value-based care environment. A background in <strong>acute care</strong> nursing and proficiency with <strong>Salesforce</strong> or other care management platforms is highly desirable.</p><p><br></p><p>Key Responsibilities:</p><ul><li>Provide remote care coordination for a panel of patients in alignment with ACO goals</li><li>Conduct telephonic outreach, health assessments, and care planning</li><li>Collaborate with interdisciplinary teams to ensure appropriate care transitions and follow-up</li><li>Identify and address gaps in care, social determinants of health, and patient barriers</li><li>Document all interactions and interventions in Salesforce or designated system</li><li>Monitor outcomes and escalate complex issues to clinical leadership as needed</li></ul><p><br></p><p>Requirements:</p><ul><li>Active RN license in <strong>New York, Texas, or Oregon</strong> (required)</li><li>Minimum 3 years of clinical nursing experience, with a preference for <strong>acute care</strong></li><li>Prior experience in <strong>ACO, care management, or population health</strong></li><li>Comfortable with technology and remote work platforms</li><li>Strong organizational and communication skills</li></ul><p><br></p><p>Preferred Qualifications:</p><ul><li>Salesforce or care management platform experience</li><li>Experience with virtual/telehealth care coordination</li><li>Bilingual skills a plus</li></ul><p></p>
