We Care Home Health is seeking a compassionate and skilled Licensed Practical Nurse (LPN) or Registered Nurse (RN) to provide specialized care for an extended care patient with trach, vent, and G-tube needs. This role involves working night shifts in the patient's home, ensuring their safety, comfort, and medical stability. The position offers a unique opportunity to deliver meaningful, one-on-one patient care while being part of a supportive community that values teamwork and open communication.

Responsibilities
• Delivering specialized care for a patient with trach, vent, and G-tube
,
• Monitoring the patient's respiratory and nutritional health during your shifts
,
• Administering medications and tube feedings
,
• Performing routine assessments, interventions, and adjustments as necessary
,
• Communicating with the care team and maintaining accurate patient records
,
• Developing strong, supportive relationships with the patient and their family

Requirements
• Current Colorado LPN or RN license
,
• Experience with trach, vent, and G-tube care preferred
,
• Current BLS/CPR certification
,
• Strong clinical and communication skills
,
• Ability to work independently in a home care setting
,
• A compassionate, patient-focused approach to nursing
