Telephone Triage Nurse (RN) - Remote at Presbyterian Homes and Services
About the position
Presbyterian Homes & Services (PHS) is seeking an on-call Registered Nurse (RN) to provide after-hours and weekend telephone triage support to our Assisted Living Communities. This remote position requires the candidate to reside in Minnesota, Wisconsin, or Iowa and to hold a current RN license or be willing to obtain licensure in these states. The role involves working from one of our PHS locations, with some flexibility in the work location. The on-call nature of this position means that the RN will be available to work during possible hours from 6:00 p.m. to 6:00 a.m. on weekdays and full days on weekends. As a Telephone Triage Nurse, you will be responsible for providing telephone response and delegation to the Resident Assistants (Home Health Aids) within the Assisted Living settings of PHS. This role requires strong competencies in clinical assessment, computer skills, and customer service. You will be expected to support regulatory and clinical practices while providing direction and clinical assessment over the phone to ensure high-quality care for residents. The ideal candidate will have a keen interest in gerontological nursing and experience in assisted living nursing or phone triage. Responsibilities
Provide telephone triage support to Assisted Living Communities after hours and on weekends.

Conduct clinical assessments over the phone to support Resident Assistants in delivering high-quality care.

Delegate tasks and provide direction to Home Health Aids based on clinical assessments.

Ensure compliance with regulatory and clinical practices in the provision of care.

Maintain accurate documentation and communication with team members regarding resident care.
Requirements
Current RN license in Minnesota, Wisconsin, or Iowa, or willingness to obtain licensure.

Experience in Assisted Living nursing and/or phone triage is preferred.

Strong clinical assessment skills and ability to provide direction over the phone.

Demonstrated ability to communicate effectively in English, both verbally and in writing.

Interest in gerontological nursing and rehabilitative/restorative clinical practices.
Nice-to-haves
Experience working in a non-profit or faith-based organization.
