Manager, Clinical Pharmacy - Transitional Care (RBE)

About the position

Oak Street Health is a rapidly growing company of primary care centers for adults on Medicare in medically-underserved communities where there is little to no quality healthcare. The Manager of Clinical Pharmacy for Transitional Care (RBE) will lead the transitional care pharmacy team as a direct manager to a team of clinical pharmacists. This position is crucial as it serves as the program owner for the clinical pharmacist portion of Oak Street and the Risk Bearing Entity (RBE)'s transitional care Medication Reconciliation Post Discharge (MRP). The manager will mentor, coach, and advise pharmacists at Oak Street Health and participate in pharmacy department strategic initiatives, program development, and pharmacist onboarding and development. The role involves developing, implementing, and managing clinical pharmacy programs for transitional care medication reviews. The manager will also develop collaborative practice agreements to support clinical pharmacy activities across multiple states, starting with Illinois, Indiana, Michigan, and Pennsylvania. A significant part of the role includes using data and analysis to track, benchmark, and communicate the outcomes of clinical pharmacy interventions. The manager will support the pharmacy team in reviewing Discharge Summaries and engaging patients, caregivers, providers, and outside pharmacies to complete medication reconciliation post-discharge. In addition, the manager will evaluate post-discharge medication orders, review medication profiles to prevent drug interactions, and ensure the appropriateness of drug therapy based on the patient's diagnosis. The role requires making recommendations to the patient's managing medical team regarding therapies or alternative approaches based on relevant medical literature and clinical experience. The manager will also be responsible for counseling patients and caregivers on proper medication administration, potential adverse reactions, and storage of medications, while participating in performance improvement activities and maintaining a culture of coaching and growth within the pharmacy team.

Responsibilities
• Directly manage a team of Clinical Pharmacists

• Develop, implement, and manage clinical pharmacy programs for transitional care medication reviews

• Develop collaborative practice agreements to support clinical pharmacy activities across multiple states

• Proactively use data and analysis to track, benchmark, and communicate the outcomes of clinical pharmacy interventions
,
• Support pharmacy team in the review of Discharge Summaries and engage patients, caregivers, providers, and/or outside pharmacies to complete medication reconciliation post discharge
,
• Evaluate post discharge medication orders and review medication profiles to prevent drug interactions
,
• Ensure the appropriateness of drug therapy based on patient's diagnosis and promote selection of cost-effective medications
,
• Contribute to patient care plans in concert with the patient's primary care physician
,
• Issue recommendations to the patient's managing medical team regarding therapies or alternative approaches
,
• Make verbal or written recommendations in a clear and appropriate manner
,
• Interview patients to determine medical history and previous medication conditions
,
• Interpret laboratory and other patient-specific data to develop treatment plans
,
• Respond to therapeutic queries posed by physicians and other healthcare providers
,
• Identify and correct drug-related problems
,
• Establish procedures for detecting significant drug interactions
,
• Ensure a continuum of care by cooperating with other health professionals
,
• Counsel patients and/or caregivers on medication administration and potential adverse reactions
,
• Participate in performance improvement activities
,
• Help coordinate and lead the onboarding of new pharmacists
,
• Conduct performance reviews and discuss team member goals

Requirements
• Licensed as a pharmacist in good standing in at least one of the states: IL, IN, MI, PA
,
• 3 or more years of experience in medication therapy management, Medicare STARS or HEDIS quality measures, managed care pharmacy, or population health
,
• Experience in clinical leadership roles, leading and coaching providers
,
• Excellent customer service skills and phone etiquette
,
• Strong working knowledge of MS Office and Google documents/spreadsheets
,
• Ability to synthesize and analyze large amounts of data to make sound business decisions
,
• Board certification (BCPS, BCACP, BCGP, etc.) and other professional degrees (e.g., M.B.A., M.P.H.) welcomed but not required
,
• US work authorization
