Job Summary
We're urgently hiring an experienced Outreach & Enrollment Specialist / Health Coach to join our team. This remote position offers an immediate start for the right candidate. As a key member of our team, you'll develop and utilize in-depth knowledge of our ImpactCare program to effectively support member engagement and quality of life. Your primary focus will be on outreach and enrollment, with a secondary emphasis on health coaching. Key Responsibilities
- Engage with existing and potential members to determine eligibility and complete enrollment, fostering awareness of our program to help change the course of chronic kidney disease and dialysis risk.
- Perform other various enrollment and disenrollment processes for related members to ensure compliance, including obtaining missing information and data to complete the enrollment process.
- Utilize and share approved educational materials in a manner that empowers and educates current and potential members.
- Appropriately document health data, including identifying potential cost containment opportunities, and refer high-risk or changes in status situations to Health Coaches and/or Clinical Care Managers.
- Partner cross-functionally with various operational business units while supporting departmental objectives within company and regulatory guidelines.
- Anticipate workflow needs based on established processes, policies, and procedures.
- Communicate your and your members' needs, challenges, and feedback to peers and Coach.
- Work collaboratively with each member, their support system, and healthcare professionals to address questions and provide additional information in a manner that supports quality health outcomes and cost-effective utilization.
- Create individualized, goal-directed, and member-centered care plans that support each member's ability to improve their health and quality of life, in conjunction with designated Clinical Care Manager.
- Autonomously create individualized, goal-directed, and member-centered health plans for lower-risk members.
- Perform a claim review with RN oversight to determine a member's clinical qualification into the program.
- Meet individually assigned metrics, including call volume and enrollment requirements, and proactively assess and revise approach as needed or directed using multiple forms of communication (i.e., phone, text, secure online chat).
- Appropriately document all outreach and health coaching efforts in a way that promotes departmental transparency and complies with HIPAA.
Requirements
Must-Haves
- At least 1 year of outbound call and/or enrollment experience.
- At least 3 years of clinical knowledge.
- An active LPN unencumbered license in good standing.
- Ability to work some evenings and/or some weekends to assist our members in various time zones.
- Bilingual in Spanish is highly preferred.
- The ability to travel multiple times a year and engage in overnight stays.
- Ability to use various peripherals (systems and equipment) simultaneously to achieve the position's Tactical Objectives and other related tasks.
- Strong working knowledge of common office software such as MS Office and Teams and ability to utilize them effectively.
- Superb written and oral communication skills, including the ability to adapt the approach and verbiage.
- Ability to effectively balance competing deadlines without losing focus on the bigger picture.
- Reliable internet and power with a designated area to conduct work with minimal interruptions.
- Learn, retain, and utilize data and information in a manner that aligns with business goals and regulatory guidelines.
Preferred Qualifications
- Experience working in a healthcare setting or with patients with chronic kidney disease.
- Knowledge of healthcare regulations and compliance.
- Ability to work in a fast-paced environment with multiple priorities and deadlines.
- Strong analytical and problem-solving skills.
- Ability to work collaboratively with cross-functional teams.
