MDS - RN, Moraga Post Acute
Moraga Post Acute is seeking a dedicated and experienced Registered Nurse (RN) for the role of Minimum Data Set (MDS) Coordinator. This position is crucial for ensuring the facility provides consistent, high-quality patient care and maintains compliance with federal and state regulations, particularly related to Medicare and Medicaid funding. The facility holds a Five-Star Medicare ranking and a High Performing rating for short-term rehabilitation.
Responsibilities:
The MDS Coordinator RN is responsible for the timely and accurate completion of MDS assessments and care plans, including:
• Coordinating the interdisciplinary team for MDS assessment completion within regulatory timeframes.
• Collecting and verifying MDS documents, ensuring data accuracy.
• Tracking and monitoring the effective utilization of PDPM tools and processes, including Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, and the PDPM Weekly Tracking Tool.
• Tracking and monitoring Quality Measure (QM) and Quality Assurance and Performance Improvement (QASP) performance monthly and quarterly, providing data analysis and recommendations for optimal outcomes.
• Providing PDPM and MDS training to new MDS Coordinators.
• Assessing the health status and level of care for all new admissions.
• Communicating level of care for new residents to all disciplines.
• Ensuring completeness and thoroughness of documentation as mandated by federal, state, and medical standards.
• Maintaining an accurate schedule of all MDS assessments with proper reference dates.
• Performing data entry and electronic submission of MDS assessments, verifying submissions and making corrections as needed.
• Scheduling and conducting resident care conferences in compliance with regulations.
• Assisting disciplines in formulating and revising care plans, ensuring problems are identified, goals are realistic, and interventions are appropriate.
• Evaluating care plans for comprehensiveness and individuality, and ensuring they are reassessed and revised appropriately.
• Managing all level of care changes within the facility, notifying departments, generating forms, and transmitting them to appropriate agencies.
• Maintaining the confidentiality of all patient care information, including Protected Health Information (PHI).

Qualifications
• Current, active, and unencumbered Registered Nurse (RN) license in good standing with the State Board of Nursing.
