Job Summary

As a Licensed Practical Nurse at Aleca Home Health, you'll utilize the nursing process to provide optimum quality and continuity of nursing care to patients in the comfort of their residence. This is a part-time position that offers a competitive salary, flexible scheduling, and opportunities for professional growth. Key Responsibilities

  • Complete and scan required record-keeping forms/charts upon patient admission, transfer, and/or discharge.
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the patient, as well as the patient's response to the care.
  • Report all discrepancies noted concerning physician's orders, diet changes, charting errors, etc., to the Director of Nursing.
  • Ensure that appropriate documentation concerning unauthorized discharges is entered in the patient's medical record in accordance with established procedures.
  • Educate patients and their families on multiple disease processes, medication management, and wound care.
  • Collaborate with advanced care providers, including nurse practitioners and physician assistants, to streamline physician orders and medical needs for patients.

Essential Qualifications

  • Current, unencumbered LPN/LVN license required or Compact State license.
  • A minimum of 2 years of LPN/LVN experience in a home health care setting.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and ability to learn electronic medical records.

Preferred Qualifications

  • Experience with electronic medical records.
  • Certification in wound care or other specialized areas.
  • Experience working with patients with complex medical needs.
