Position Overview

As a full-time Remote RN Case Manager, you will play a vital role in our home health and hospice care team, working with patients and their families to develop and implement personalized plans of care. This is a unique opportunity to make a meaningful impact on the lives of patients and their families, providing much-needed care in the comfort of their own homes.

Key Responsibilities

As a RN Case Manager at Amedisys, your key responsibilities will include: * Performing patient assessments and collaborating with the care team to develop and implement a plan of care * Making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so * Promoting patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques * Supervising LPNs and HHAs * Completing documentation timely and accurately * Regularly communicating patient progress to the clinical manager and care team * Planning and providing staff education * Performing on-call responsibilities and on-call services to patients/families as assigned * Participating in clinical development and continuing education programs * Other duties as assigned

Qualifications

To be successful as a RN Case Manager at Amedisys, you will need: * One (1+) year of clinical experience as a Registered Nurse (RN) * Current RN license, specific to the state(s) you are assigned to work * Current CPR certification * Valid driver's license, reliable transportation, and liability insurance * If less than 6 months clinical experience as a RN, candidate must participate in RN Intern program
