As a Clinical Case Manager, you will play a vital role in our primary care clinic, working directly with patients to identify their needs and develop personalized care plans. Primary Responsibilities

  • Patient Care and Support: Communicate with patients, responding to their questions and concerns via patient portal and other modalities. Perform assessments of health conditions and implement care plans in collaboration with patients, caregivers, clinicians, and other healthcare professionals.
  • Care Coordination: Pursue interventions to reduce the risk of condition exacerbation, ER, and hospitalization utilization. Perform medication reconciliation and collaborate with clinician partners as needed.
  • Patient Education and Empowerment: Conduct Motivational Interviewing and Self-Management Goal setting. Provide patient education and create referrals to appropriate agencies and resources.
  • Transition of Care: Support the transition of care from Emergency Department or inpatient stay to outpatient setting. Perform assessments of transitional needs, medication reconciliation, and establish contingency plans.
  • Quality Measures and Reporting: Achieve Quality Measures outcomes via reduction in HEDIS Gaps in Care. Perform accurate and timely documentation in the electronic medical record and prepare reports as required.
  • Collaboration and Teamwork: Participate in daily huddles and monthly clinic meetings. Serve as a facilitator and resource for other members of the Medical Group clinical team.
  • Professional Development: Maintain continued competence in nursing practice and knowledge of current evidence-based practices. Attend departmental meetings and provide constructive recommendations for process improvement.

Essential Qualifications

To be successful in this role, you will need to meet the following essential qualifications:

  • Associate Degree in Nursing: A minimum of an Associate degree in nursing is required.
  • Valid RN License: A valid NM RN License or valid multi-state compact license is required.
  • Current BLS Certification: Current BLS Certification is essential for this role.
  • Age and Driver's License: You must be 21 years or older and have a valid NM State Driver's license.
  • Previous Experience: 3+ years of previous job-related experience in a healthcare environment is required.
  • Knowledge of Medical Terminology: You should have a strong knowledge of medical terminology.
  • MS Office Software Applications: You should be skilled with MS Office software applications.
  • Full COVID-19 Vaccination: Full COVID-19 vaccination is an essential job function of this role.

Preferred Qualifications

While not essential, the following qualifications are highly desirable:

  • Experience in Case Management: Previous experience in case management is highly preferred.
  • Knowledge of Evidence-Based Practices: Knowledge of current evidence-based practices is essential for success in this role.
  • Excellent Communication Skills: Excellent communication and interpersonal skills are critical for effective collaboration with patients, caregivers, and healthcare professionals.
