About the position

Responsibilities

  • Execute nursing interventions by collaborating with the care team and following established standards of care.
  • Assess and collect data related to patient health status and develop care plans with the care team.
  • Utilize data to meet care coordination needs across settings in conjunction with patients and families.
  • Manage nursing care for individual patients, ensuring accountability and responsibility.
  • Document nursing assessments and interventions comprehensively.
  • Identify patient and family learning needs and educate them using the teach-back method.
  • Provide safe, ethical, and evidence-based patient care that is culturally congruent.
  • Perform telephone, My Chart, and in-person triage for symptom management.
  • Review and act on test results and manage communications requiring RN education and training.
  • Provide patient education through various channels including in-person, telephone, or video.
  • Assist with medication management and complete timely documentation in the electronic medical record.
  • Manage urgent and emergent patient care situations and participate in care coordination activities.
  • Establish standard clinical supply storage and manage medication/immunization inventory.
  • Float to other clinics as needed and serve on relevant committees and task forces.

Requirements

  • Bachelor's degree in Nursing or equivalent education, training, and experience.
  • Four years of related experience.
  • Current and unrestricted New York State RN license.
  • BLS certification.
  • Ability to work in an environmentally-controlled indoor setting.
  • Ability to perform sedentary work involving long periods of sitting or standing.
  • Ability to travel between clinical sites as needed.
  • Exposure to blood, bodily fluids, and tissue, with the ability to perform emergency care as needed.
  • Excellent communication skills and ability to work collaboratively with a diverse population.
  • Strong proficiency in computer skills including Electronic Health Systems and Microsoft Office.

  • Physician practice experience preferred.

Benefits

  • Competitive salary range of \$98,000 - \$107,000 based on qualifications and experience.
  • Regular work schedule of 40 hours per week, Monday to Friday, 8 AM to 5 PM.
